forehead acne ,pimples & pigmetation

i have lots of forehead acne.
before 4-5 month my face is clean , but in between 3-4 month my face is full of forehead acne & pimples pigmentation on cheeks &outside the mouth.
lot many black heads on nose.
how i treat it?
