Dear Parent,
Your concern is understandable. At 4 years of age, a child is generally expected to develop the ability to chew different food textures. If your child swallows food directly without chewing and refuses fruits and almost all solid foods, it needs proper evaluation.
Possible reasons include:
1. Oral-motor difficulties – inadequate coordination or strength of the tongue, lips and jaw muscles.
2. Sensory food aversion – difficulty accepting different food textures.
3. Delayed development of chewing skills.
4. Dental, oral or swallowing-related problems.
5. Behavioural feeding difficulties or previous unpleasant eating experiences.
**What should you do?**
- Consult a paediatrician for a detailed assessment.
- Consider an evaluation by a speech and language therapist experienced in paediatric feeding and swallowing.
- Do not force the child to chew or introduce hard foods suddenly.
- Gradually introduce age-appropriate food textures under professional guidance.
- Monitor weight, growth and nutritional status.
If the child coughs, chokes, gags frequently, has a wet voice while eating or has recurrent chest infections, swallowing assessment is particularly important. Breathing difficulty or choking requires emergency care.
**A few important questions:**
- Does the child chew biscuits, bread or any other food at all?
- Does the child swallow even large pieces of food directly?
- Is speech and other developmental progress normal?
- Is the child's weight increasing normally?
These details will help understand the problem better.
