What is Prostate Laser Surgery?

Prostate laser surgery is a modern technique used to treat benign prostatic hyperplasia (BPH), which is an enlarged prostate condition. As men get older, the prostate gland can grow bigger and press on the urethra, which carries urine out of the body. This can lead to problems such as weak urine flow, difficulty starting or stopping urination, frequent need to go, and poor bladder control.

In the previous times, open surgery was the main option for treating severe prostate enlargement. But in today’s time, prostate laser surgery is a very good alternative for this cause. Using a special laser, surgeons can remove or shrink extra prostate tissue that blocks the flow of urine. This reduces symptoms, improves comfort, and helps men regain control of their bladder function. Patients often prefer laser surgery because it typically means less bleeding, shorter hospital stays, and faster healing compared to conventional surgery.

Who May Need Prostate Laser Surgery?

Prostate laser surgery may be recommended for men who:

- Have bladder outlet obstruction due to an enlarged prostate

- Experience frequent urinary urgency or inability to empty the bladder

- Have a weak urine stream that affects daily life

- Develop urinary retention (inability to urinate)

- Have repeated urinary tract infections or bladder stones

- Cannot tolerate medication or have side effects from treatment

Types of Prostate Laser Surgery

There are several types of prostate laser surgery, chosen based on individual needs and prostate size:

1. GreenLight Laser Therapy: Uses a high-powered laser to vaporize excess prostate tissue. It’s known for minimal bleeding and quick recovery.

2. Holmium Laser Enucleation (HoLEP): Removes large portions of prostate tissue and is effective for very enlarged prostates.

3. Thulium Laser Surgery: Offers precise cutting and resection with less irritation to surrounding tissue.

4. Photoselective Vaporization (PVP): This laser technique vaporizes obstructing tissue and is often suitable for moderate prostate enlargement.

Your doctor will recommend the best laser option based on how large your prostate is and your overall health.

Prostate Laser Surgery Procedure

Before the procedure, your doctor will review your medical history, evaluate your symptoms, and perform some tests like ultrasound or urodynamic studies. You may be advised not to eat or drink for a few hours before surgery. Prostate laser surgery is usually done under general or spinal anesthesia, so you will be asleep or numb from the waist down and feel no pain.

During the procedure:

1. A thin, flexible tube called a cystoscope is inserted through the urethra (without any external cuts).

2. The laser fiber is passed through the cystoscope to reach the prostate.

3. The surgeon uses laser energy to vaporize or remove the excess prostate tissue that blocks urine flow.

4. Once enough tissue is treated, the instruments are removed.

5. A temporary urinary catheter may be placed to help drain urine.

The laser targets only the excess prostate tissue, not surrounding nerves. Because of the precision, there is less bleeding and fewer complications compared to traditional surgery.

Recovery & Aftercare

After prostate laser surgery, you will be moved to a recovery area and monitored for a few hours. A urinary catheter is often left in place for a short time to help your bladder empty. Mild discomfort and irritation are common immediately after surgery, but pain can usually be controlled with medications.

While recovering for prostate laser surgery, keep these tips in mind:

- Drinking plenty of water to flush the bladder

- Avoiding heavy lifting and strenuous exercise for a few weeks

- Following all instructions for wound and catheter care

- Taking prescribed antibiotics or pain relief medicines as recommended

- Reporting any fever, severe pain, or bleeding to your doctor

Most men can leave the hospital within a day or two, and many notice improvement in symptoms within a few days to a few weeks. Full recovery takes time, but most men enjoy better urine flow and fewer urinary symptoms after healing.

Risks & Possible Complications

Prostate laser surgery is generally safe, but like all surgeries, it carries some risks. These may include:

- Temporary burning sensation during urination

- Blood in urine (usually mild and short-term)

- Urinary tract infection

- Bladder spasms or urgency

- Urethral strictures (scar tissue narrowing the urethra)

- Rare injury to surrounding tissues

Most complications are mild and treatable. Your healthcare team will explain all risks before surgery and help you understand how to minimize them.
