What Is Odontectomy and How Is It Performed?

Odontectomy is a minor oral surgical procedure used to remove an impacted or difficult-to-access tooth, most often the wisdom teeth (third molars) that have not erupted normally into the mouth.

Unlike a simple “pulling” of a fully visible tooth, odontectomy involves carefully opening the gum and often removing a small portion of bone around the tooth so that the surgeon can safely extract it. In many cases the tooth may need to be divided into smaller pieces to remove it with less trauma.

Wisdom teeth are the last to grow, usually appearing in the teens and early twenties, and often don’t have enough space to come in straight. When this leads to pain, infection, or damage to nearby teeth, an odontectomy is often recommended to prevent or fix these issues.

Purpose & Benefits of Odontectomy

● Removes impacted or stuck teeth that cannot erupt normally.

● Relieves pain, swelling, and infection caused by impacted wisdom teeth.

● Prevents damage to neighbouring teeth and gum tissue.

● Helps improve oral hygiene and reduces food trapping behind wisdom teeth.

● Reduces the risk of cysts or other complications around an impacted tooth.

● Can restore mouth comfort and function (better chewing and less jaw tension).

Who May Need Odontectomy?

Odontectomy may be recommended for people who:

● Have impacted teeth that are stuck under the gum or bone and cannot erupt properly.

● Experience recurrent pain, swelling, or gum infection (pericoronitis) around a wisdom tooth.

● Have tooth decay or damage in wisdom teeth that cannot be fixed with fillings.

● Are at risk of damage to adjacent teeth due to pressure from a growing wisdom tooth.

● Need to prepare for orthodontic treatment or to make room for other teeth.

● Experience jaw discomfort, headaches, or restricted mouth opening due to impacted teeth.

A dental surgeon will assess your symptoms, X-rays, and overall oral health to decide whether odontectomy is the best choice for your situation.

Types of Odontectomy

Standard Odontectomy

This is the full surgical removal of an impacted tooth, including creating an incision in the gum, clearing bone if needed, and removing the entire tooth.

Partial Odontectomy (Coronectomy)

In some cases, especially when a lower wisdom tooth is very close to a major nerve, only the crown (top) of the tooth is removed while the roots are left in place to reduce nerve injury risk.

This technique, known as coronectomy or partial odontectomy, helps protect vital nerves and can reduce complications while still relieving problems caused by the tooth’s crown.

Odontectomy Procedure: Step-by-Step

1. Consultation and Preparation: Your surgeon reviews your medical and dental history and examines X-rays to see the position of the impacted tooth and any nearby nerves.

2. Anesthesia: Odontectomy is usually done under local anaesthesia (numb area) or sedation, depending on complexity and patient comfort.

3. Incision: A small cut is made in the gum tissue to expose the tooth. Sometimes a tiny bit of bone covering the tooth is also removed by the dentist.

4. Tooth Sectioning: If the tooth is difficult to remove in one piece, it is divided into smaller sections so it can be taken out gently.

5. Extraction: The treated tooth (or sections) is carefully removed, and the socket is cleaned to make sure no debris is left behind.

6. Closure: The gum tissue is repositioned and stitched if needed. Your surgeon may use dissolvable stitches that don’t require removal later.

The duration of the procedure varies, but most odontectomies are completed in about 30 minutes to an hour, depending on complexity.

Recovery & Aftercare

After odontectomy:

● Mild to moderate discomfort and swelling around the surgery site is common and typically improve in a few days.

● You may receive pain-relieving medicine and instructions on how to keep the area clean.

● Gentle warm salt-water rinses may be recommended to promote healing.

● Avoid smoking, using straws, or vigorous rinsing for a few days to help protect the blood clot in the socket.

● Soft foods and good oral hygiene help speed recovery.

Most people begin to feel better within a week or two, though complete healing of the bone can take longer.

Risks & Possible Complications

Odontectomy is generally safe, but as with any surgical procedure, there are some possible risks:

● Pain and swelling at the surgical site.

● Infection if bacteria enter the wound.

● Bleeding shortly after surgery.

● Dry socket, where the protective clot dislodges and exposes bone.

● Nerve irritation or numbness in rare cases, especially with lower wisdom teeth.

● Damage to adjacent structures if the tooth is very close to nerves or other teeth.

Your surgeon discusses specific risks with you before the procedure and explains how to reduce them.

Odontectomy vs Other Tooth Removal Methods

● Odontectomy vs Simple Extraction: A simple extraction is for a fully erupted and easily accessible tooth. Odontectomy is used when the tooth is impacted or difficult to reach and requires surgical access.

● Odontectomy vs Coronectomy: Full odontectomy removes the whole tooth, while coronectomy (partial odontectomy) removes only the crown to reduce risk to nearby nerves.

Your dental surgeon recommends the best option based on tooth position, anatomy, and overall health.

Frequently asked questions

What is odontectomy used for?

Odontectomy is used to remove impacted or difficult-to-access teeth, especially wisdom teeth that are painful, infected, or causing other problems.

Is odontectomy painful?

You will not feel pain during the procedure because of anaesthesia. Some mild pain and swelling afterward is normal but can be managed with medicines.

How long does an odontectomy take?

The surgery usually takes about 30 minutes to an hour, depending on the tooth’s position and difficulty.

Will I stay in the hospital?

In most cases, you can go home the same day after the procedure, as it is commonly done on an outpatient basis.

How long is recovery?

Most people feel better in about a week or two, though full healing of the surgical site can take longer.

Can I eat normally after an odontectomy?

You should start with soft foods and avoid hard or chewy foods until healing advances.

Is odontectomy safe?

Yes, when performed by trained professionals, odontectomy is generally safe, though your dentist will discuss specific risks and aftercare with you.
