Who Needs ICSI Treatment?

Intracytoplasmic Sperm Injection (ICSI) is an advanced fertility treatment used to help couples who have difficulty conceiving naturally. It is a specialised form of in vitro fertilization (IVF) in which a single sperm is injected directly into an egg in a laboratory to achieve fertilization. The technique was developed to treat male infertility problems, such as low sperm count, poor sperm movement, or abnormal sperm shape. By directly injecting a healthy sperm into the egg, ICSI helps overcome many barriers that prevent natural fertilization.

During a normal fertilization process, thousands of sperm must swim to reach and penetrate the egg. In some cases, sperm are unable to reach or fertilize the egg due to quality or movement issues. ICSI bypasses this problem by manually placing a sperm inside the egg using specialized microscopic tools. ICSI is usually performed as part of an IVF treatment cycle. Eggs are collected from the ovaries, fertilized in a laboratory using the ICSI technique, and the resulting embryo is then transferred into the uterus.

This treatment has helped many couples with infertility achieve pregnancy. It is widely used in fertility clinics around the world and has become one of the most effective treatments for severe male infertility.

Who May Need ICSI?

Doctors may recommend intracytoplasmic sperm injection for couples experiencing infertility, particularly when sperm-related issues are present.

Common reasons for ICSI include:

● Poor sperm motility (movement)

● Abnormal sperm shape

● Sperm retrieved surgically from the testes or epididymis

Steps Involved in the ICSI Procedure

The ICSI procedure involves several stages and is usually performed in a fertility clinic.

1. Ovarian Stimulation

The female partner receives fertility medications to stimulate the ovaries to produce multiple eggs instead of just one. Doctors monitor the development of eggs using ultrasound scans and blood tests.

2. Egg Retrieval

When the eggs are ready, the doctor collects them from the ovaries through a minor surgical procedure called egg retrieval. This procedure is usually done under mild sedation.

3. Sperm Collection

A sperm sample is collected from the male partner. In some cases, sperm may be surgically retrieved from the testes if they are not present in the semen.

4. Sperm Injection

In the laboratory, an embryologist selects a healthy sperm using a microscope. The sperm is then injected directly into the centre of the egg using a very fine needle.

5. Fertilization and Embryo Development

After injection, the eggs are placed in a special incubator where fertilization and embryo development are monitored for several days.

6. Embryo Transfer

Once healthy embryos develop, one or more embryos are transferred into the uterus to attempt pregnancy.

7. Pregnancy Test

About two weeks after embryo transfer, a pregnancy test is performed to determine whether the procedure was successful.

Success Rates of ICSI

The success of ICSI depends on several factors, including the age of the woman, egg quality, sperm health, and the expertise of the fertility clinic.

Research shows that fertilization occurs in about 65–80% of mature eggs using the ICSI technique.

Pregnancy success rates are similar to traditional IVF procedures, although ICSI significantly improves fertilization chances when male infertility is involved.

Younger women generally have higher success rates because egg quality tends to decrease with age.

Recovery and Aftercare

Recovery from ICSI depends mainly on the IVF procedures involved, particularly egg retrieval.

After egg retrieval and embryo transfer:

● Mild cramping or bloating may occur

● Light spotting is sometimes experienced

● Doctors may prescribe hormone medications to support pregnancy

● Patients should avoid heavy physical activity for a few days
