When is a Pulpotomy Needed?

Pulpotomy is a dental procedure used to treat and save a tooth with infected or inflamed pulp (the inner soft tissue). It is commonly performed when tooth decay reaches the pulp inside the crown (visible part) of the tooth, but has not spread to the roots.

During a pulpotomy, the dentist removes the damaged pulp from the crown of the tooth while leaving the healthy pulp in the roots intact. This helps preserve the tooth’s structure and function.

This procedure is most often used in children’s primary (baby) teeth, but it can also be performed in some adult cases when the pulp is still healthy in the root area.

Pulpotomy is often referred to as a “baby root canal”, as it is less invasive than a full root canal treatment and helps maintain the natural tooth until it is replaced by a permanent tooth.

Who May Need a Pulpotomy?

A dentist may recommend pulpotomy for patients who have:

● Deep tooth decay (cavities) reaching the pulp

● Inflamed or infected pulp (pulpitis)

● Tooth damage due to injury or trauma

● Pain or sensitivity to hot, cold, or sweet foods

● Exposure of pulp during cavity removal

● Primary teeth that need to be preserved until permanent teeth erupt

Pulpotomy is usually recommended when the infection is limited to the crown and the root pulp is still healthy.

Types of Pulpotomy

There are different types of pulpotomy depending on how much pulp is removed.

1. Partial Pulpotomy

Only a small portion of the infected pulp is removed. This is used when the damage is minimal and limited.

2. Full Pulpotomy

The entire pulp in the crown is removed while keeping the root pulp intact.

3. Medicament-Based Pulpotomy

After removing the pulp, the dentist places a medicated dressing such as calcium hydroxide or mineral trioxide aggregate (MTA) to protect the remaining pulp and promote healing.

The choice of type depends on the severity of decay and pulp condition.

Pulpotomy Procedure

A pulpotomy is usually performed in a dental clinic and takes about 30–45 minutes.

1. Examination and Diagnosis

The dentist evaluates the tooth using clinical examination and X-rays to confirm that pulpotomy is suitable.

2. Local Anesthesia

The affected area is numbed using local anesthesia to ensure a painless procedure.

3. Removal of Decay

The dentist removes the decayed portion of the tooth and creates a small opening to access the pulp.

4. Removal of Infected Pulp

The infected pulp in the crown is carefully removed while preserving the healthy pulp in the roots.

5. Placement of Medication

A medicated material is placed over the remaining pulp to protect it and promote healing.

6. Sealing and Restoration

The tooth is sealed with a filling or dental crown to restore its function and prevent further infection.

Recovery & Aftercare

Recovery after pulpotomy is usually quick, and most patients can return to normal activities the same day.

After the procedure, patients may experience:

● Mild pain or discomfort

● Temporary numbness from anesthesia

● Slight swelling or sensitivity

These symptoms usually improve within a few hours to a day.

Aftercare tips include:

● Avoid eating until numbness wears off

● Eat soft foods for a short time

● Maintain good oral hygiene

● Follow the dentist’s instructions for medications

● Attend follow-up visits if required

Risks & Possible Complications

Pulpotomy is generally a safe and effective procedure, but some risks may occur.

Possible complications include the following:

● Persistent pain or sensitivity

● Failure of the pulp to heal

● Infection spreading to the root

● Need for further treatment such as root canal or extraction

The success of the procedure depends on early diagnosis and proper dental care.

Frequently asked questions

What is pulpotomy?

Pulpotomy is a dental procedure where the infected pulp from the crown of a tooth is removed while keeping the root pulp intact.

Is pulpotomy painful?

No, the procedure is performed under local anesthesia, so patients usually do not feel pain during treatment.

How long does a pulpotomy take?

The procedure typically takes 30–45 minutes, depending on the case.

Is pulpotomy the same as a root canal?

No, pulpotomy removes only the pulp in the crown, while a root canal removes pulp from both the crown and roots.

How long does recovery take after pulpotomy?

Most patients recover within a day, although mild discomfort may last for a short time.

Can adults undergo pulpotomy?

Yes, but it is more commonly performed in children. Adults usually require a root canal if the infection is extensive.

What happens if pulpotomy fails?

If the procedure is unsuccessful, further treatment such as a root canal or tooth extraction may be needed.
